Forms

• Computer Account (Renewal) Request Form - Please use this form to apply for computer accounts on the EE Labs and Network. This form will generate a PDF file, which you will have to print and bring it in to B20-106A, with appropriate signatures.
• Laboratory Use Authorization - To Authorize individuals for Lab access when they are normaly closed. (PDF Form)
• Senior Project Parts Form - To request parts (IC's only) for Senior Project use. (PDF Form).
• State Equipment Use Request - Checkout of state equipment for individual use. (PDF Form)
